Are file changes checked based on modified date or something else?

I have a folder containing a ton of files (probably over a million), most of which will not change, save maybe because of corruption on the disk. From what I understand, indexing causes all the contents of all the files to be read. This takes forever. Is there any way to, say, reindex files only based on their last modified date? Edit: Just realized that wouldn't work, given BTSync is based on the BT protocol.
